Streamlined Enrollment Process Flowchart

Uninsured child arrives at CHC, private provider, urgent care clinic, ER, school nurse’s office, etc.

Child’s parent is asked whether he/she would like to apply for health coverage; if yes, consent form is provided to complete; also provided is an informational flyer; (3 programs - Healthy Families, Medi-Cal & Healthy Kids).

Daily, office manager/nurse/other batches forms and sends to DSS.

DSS receives consent forms and assigns to case manager to call family (application completion, questions, etc.)

Case manager screens for free Medi-Cal; if not eligible, screens for Healthy Families; if not eligible, screens for Healthy Kids.
